The H.S.E. Vaccination Team will be with us on Monday 19th November from 9.30 a.m.
Please note that all consent forms must be completed and returned to me (in the enclosed envelope) whether or not your child requires the vaccinations.
On the day of the vaccinations the children are to wear their PE tracksuits. They must also have eaten breakfast and they must bring their immunisation record card/passport with them.
Parents are welcome to accompany their child on the day.
NB: If your child has already received the vaccines, or you do not wish for your child to receive these vaccines, please sign (Part 4)and return the form to me ASAP.
